New Patient Registration

Anyone in the UK can register with us. However, only patients who live within the following areas and who meet the clinical criteria will be able to receive a home visit:

Ernesettle, West Park, Barne Barton, St Budeaux, Trelawny, North Prospect, Milehouse, Penny Cross, Mannamead, Beaumont, Peverell, Hartley, Mutley, Lipson, Greenbank, Mount Gould, and Efford.

(The above covers the areas: PL2 2, PL2 3, PL3 4, PL3 5, PL3 6, PL3 7, PL4 6, PL4 7, PL4 8, PL4 9, PL5 1, PL5 2, PL5 3, PL5 4 - Whitleigh only, PL5 9)

If you reside in any other area of Plymouth not included above, you may still be eligible to register with our practice; however, you will not qualify for home visits. Please contact us to find out more.

There are 3 ways to register at the practice:

1. Attend in person and complete a registration form

2. Download a registration form here and either email it to us or post it to us

3. Complete an online registration form by clicking here

Please note: If you are already registered as a patient at our surgery, you do not need to register again to use our smartphone app or to submit an eConsultation or online consultation.
